Do I need a referral?
Many insurance companies prohibit physicians from treating a patient without an appropriate referral. If your plan requires a referral, you will need to have it called into NMHI prior to an appointment or brought with you at the time of your visit. Please contact our office for assistance with your specific insurance plan requirements.
What is your cancellation policy?
If you are unable to keep a scheduled appointment, please notify our office 24 hours prior to your appointment. If you may be late for a scheduled appointment, please contact the office as soon as possible.
Do you have parking?
Each office has convenient parking available. If you are visiting our Downtown Albuquerque office, patient and vendor parking is available by entering our facility off Elm Street and driving to the second level on the eastside of the building facing I-25. Patients park and enter the building on the second level of the building.
How do I make an appointment?
Patients are seen by appointment Monday through Friday. Contact the office most convenient for you and NMHI staff will schedule an appointment. You may call 800-715-6644 or 505-841-1000.
What are your hours of operation?
Our main facility located in Downtown Albuquerque is open 8:00 a.m. - 5:00 p.m. Monday through Friday; however, other NMHI locations may hold varying office hours. Please visit the our location section for specific hours of operation.
